NAD+ dosage: injection doses, vial strengths and what the studies used

There is no standard NAD+ injection dose. No NAD+ drug is FDA-approved, so no label sets one, and FDA still lists NAD+ as a compounding substance under evaluation. Human data on NAD+ injected under the skin comes from one preprint that gave 100 mg a day for three days to six people. The best-known human study gave 750 mg through an IV drip over six hours. Of the 10 online sellers we checked, only AgelessRx publishes a dose per injection: start at 20 mg and slowly work up to 80 mg, one to three times a week, from a 200 mg per mL vial. The others publish a frequency, a vial or monthly total of 500 to 1,000 mg, or nothing at all. Your prescriber sets your dose. Ask for it in milligrams per injection.

Is there a standard NAD+ injection dose?

No. FDA's approved-drug data held no NAD+ product when we searched it on October 17, 2026, so no FDA label sets a dose. FDA's compounding list, updated May 14, 2026, places NAD+ among bulk substances still under evaluation for section 503A compounding. While a substance sits in that category, FDA says it does not intend to act against compounders who use it under stated conditions. That is an enforcement policy, not an approval.

A 2026 systematic review found no outcome trials of IV or intramuscular NAD+ itself for anti-ageing or wellness uses. The doses in use come from prescriber judgment and seller protocols, not from trials that compared doses for a health result.

What NAD+ doses have human studies used?

Only two studies gave NAD+ itself. The 2019 IV pilot gave 750 mg over six hours to eight men, once. A 2026 preprint, not yet peer-reviewed and funded by ChromaDex, which makes a nicotinamide riboside ingredient the study also tested, gave 100 mg a day for three days, under the skin to six people, into muscle to four and by vein to five. It mainly measured injection discomfort.

The larger and longer trials tested oral precursors, nicotinamide riboside and NMN, at 100 to 1,000 mg a day for six to ten weeks. The body converts those molecules into NAD+, but they are different products taken by mouth, so their doses do not carry over to an injection.

Do online NAD+ sellers publish a dose?

Only one of the 10 does. AgelessRx is the only seller that publishes a dose per injection. It tells new users to start with 0.1 mL, which is 20 mg, and slowly work up to 0.4 mL, or 80 mg, one to three times a week, from a 200 mg per mL vial sold in 500 mg and 1,000 mg sizes. Pallas describes a weekly injection with clinician-personalized dosing, and Willow says its physician sets the dose and frequency.

Sprout Health says many patients start with a short loading phase, then take maintenance doses often one to two times a week, but gives no milligrams per dose. The rest publish a strength or a total, or nothing. AgelessRx and PeterMD state 200 mg per mL, and Wellorithm's 500 mg in 2.5 mL works out the same. HealthRX, Sprout Health and System Labs advertise 1,000 mg without saying how much goes into each injection. Embody lists 500 mg to 1,000 mg injections without saying whether that is per vial or per month.

How long does a 1,000 mg NAD+ vial last?

Divide the vial's milligrams by your weekly total. Across AgelessRx's published range, the weekly total runs from 20 mg, one 20 mg injection, to 240 mg, three 80 mg injections. At those totals a 1,000 mg vial would last from about four weeks to about 50 weeks. That is our arithmetic from its page, not a schedule. AgelessRx also says an opened vial must be discarded after 28 days, so at low doses much of a 1,000 mg vial goes unused. Follow the discard date on your pharmacy label.

When a seller sells 1,000 mg as one month's supply, as HealthRX does, using it all in a month means about 230 mg a week, close to the top of AgelessRx's range. PeterMD sells two 200 mg per mL vials as an eight to ten week supply without stating the vial volume.

How much NAD+ is in 0.1 mL?

At 200 mg per mL, the strength three sellers state or imply, 0.1 mL holds 20 mg and 0.5 mL holds 100 mg. AgelessRx's instructions match that: 0.1 mL for 20 mg and 0.4 mL for 80 mg. A vial at a different strength changes the arithmetic, so read the concentration on your pharmacy label.

AgelessRx suggests its 2.5 mL vial for new users or anyone taking up to 50 units a week, and the 5 mL vial for higher weekly doses. Its dose instructions use mL while that vial advice uses units, so check which measure your prescription uses.

Are IV, injection and oral NAD+ doses comparable?

No. The 750 mg IV dose ran into a vein over six hours in a supervised study. Injections under the skin are tens of milligrams at a time, and we found no published human data on how much of an injected dose reaches the blood. Embody's page claims about 100% bioavailability for its injections without citing a source.

Oral nicotinamide riboside and NMN are precursors taken in hundreds of milligrams a day.

Does a higher NAD+ dose work better?

For oral precursors, higher doses raised blood NAD+ further. In a 140-person trial, 100, 300 and 1,000 mg of nicotinamide riboside a day raised whole-blood NAD+ by 22%, 51% and 142%, and an NMN trial found the highest blood levels at 600 to 900 mg a day. Those are blood measurements, not health outcomes, and both trials had industry ties.

No study has compared NAD+ injection doses for any result, so there is no evidence that a higher injected dose works better. Our side effects guide covers reported reactions to injectable NAD+.

What should you ask before starting NAD+ injections?

Ask for four numbers in writing: milligrams per injection, injections per week, the vial's strength in mg per mL, and how many weeks the vial should last. Ask whether you start low and increase, as AgelessRx describes, and who adjusts the dose if you react.

Compounded NAD+ is not FDA-approved, and FDA has reminded compounders to use ingredients suitable for sterile injection after adverse event reports. Ask which pharmacy prepares your vial.
